Endometriosis-Associated Infertility

Endometriosis is a chronic, estrogen-dependent disease that affects up to 10% of women of reproductive age and is strongly associated with impaired fertility. The mechanisms underlying endometriosis-associated infertility are multifactorial, involving anatomical distortion, an altered peritoneal environment, impaired tubal and ovarian function, and reduced endometrial receptivity. Advances in pathophysiological understanding have highlighted the complex interplay between inflammation, angiogenesis, neurogenesis, and hormonal imbalance in shaping the reproductive outcomes of affected women.

Accurate diagnosis represents a cornerstone of the disease’s clinical management. While laparoscopy remains the historical gold standard, non-invasive imaging techniques—particularly transvaginal ultrasound—have emerged as essential tools for detecting ovarian and deep-infiltrating lesions, assessing disease extent, and guiding therapeutic decisions.

This webinar aims to provide an updated and evidence-based overview of the relationship between endometriosis and infertility. We will discuss the chances of spontaneous conception, the role and outcomes of assisted reproductive technologies, and the impact of both medical and surgical treatments before in vitro fertilization (IVF). The goal is to offer clinicians and researchers practical insights for individualized patient care and to highlight current challenges and future perspectives in this evolving field.

Simone Ferrero obtained his specialization in Gynecology and Obstetrics at the University of Genoa (Genoa, Italy) and the title of Doctor of Philosophy (Ph.D.) at Queen Mary Hospital (London, UK). He is currently an Associate Professor of Gynecology and Obstetrics at the University of Genoa, and works at IRCCS San Martino Policlinic Hospital (Genoa, Italy). Simone Ferrero is the author of over 400 publications in scientific journals indexed in PubMed with impact factors. He has already been an editor of international books about benign gynecological diseases, and he is a member of several editorial boards of international gynecological journals.

Fabio Barra, MD, is an obstetrician and gynecologist at IRCCS Ospedale Policlinico San Martino, Genoa, Italy. His main expertise includes the ultrasound diagnosis and both medical and surgical management of benign gynecological diseases, with a special focus on endometriosis, as well as gynecologic oncology.

Anna Stepniewska is a gynecologist from Italy. She works in a referral center for endometriosis, Negrar Hospital (Verona), which is directed by Prof Ceccaroni. Since 2000, she has been involved in the assistance of patients with endometriosis, from diagnosis to surgical and medical treatment, and in research in this field. She has performed numerous surgeries for deep endometriosis, including complex cases, as a first surgeon and numerous ultrasound scans for endometriosis.

Dr. Irene Gazzo is an Obstetrician and Gynecologist, currently serving as a Medical Doctor at the Center for Reproductive Medicine of Evangelical International Hospital in Genoa, Italy. She undertook her residency in Obstetrics and Gynecology at the University of Genoa and has obtained a Master’s degree in Andrological Endocrinology, Reproductive Medicine and Sexual Disorders from the University of Padua. Her main clinical and scientific interests include male and female infertility, fertility preservation, endometriosis, and the impact of oxidative stress on reproductive outcomes. She has authored several publications in international peer-reviewed journals, contributed as speaker and poster presenter at major national and international congresses, and was selected as an ESHRE Young Ambassador in recognition of her early-career achievements.

Andrea Busnelli is an Assistant Professor of Obstetrics and Gynaecology at Humanitas University, Milan, Italy. He received his medical degree from the University of Milan in 2012, where he also completed his residency program in obstetrics and gynecology and obtained a PhD in Public Health. His main research interest is reproductive medicine.

Associate Professor in the Unit of Obstetrics and Gynecology, “Paolo Giaccone” Hospital, Department of Health Promotion, Mother and Child Care, Internal Medicine and Medical Specialties (PROMISE), University of Palermo (Palermo, Italy). Antonio Simone Laganà was born in Reggio Calabria (Italy) on 8th May 1986. In the past, he acted as Coordinator of the Special Interest Group for Endometriosis & Endometrial Disorders (SIGEED) of the European Society of Human Reproduction and Embryology (ESHRE). Antonio Simone Laganà was trained in obstetric/gynecological ultrasound at the Altamedica Main Centre in Rome (Italy), obtained a Master’s degree in “Gynecological Minimally Invasive and Robotic Surgery” at the University of Pisa (Italy) and a Ph.D. in “Medical and Surgical Biotechnologies” at the University of Messina (Italy), and underwent intensive training in minimally invasive gynecological surgery (high-volume advanced laparoscopy, hysteroscopy) at the Univerzitetni Klinični Center Ljubljana (Slovenia) during a Subspecialist Training Programme (Fellowship) in Reproductive Medicine (according to the criteria of the European Society of Human Reproduction and Embryology, ESHRE, and European Board and College of Gynecology and Obstetrics, EBCOG). His research interests include endometriosis, reproductive immunology, infertility, gynecological endocrinology, laparoscopy, and hysteroscopy. He is the author of more than 580 papers published in PubMed-indexed international peer-reviewed journals, and his presence is often requested as an invited speaker at international congresses. He is currently an editor of several high-impact journals, including Journal of Minimally Invasive Gynecology, Gynecologic and Obstetric Investigation, and many others.
